Why Does BOTOX® Work Only on Certain Wrinkles and Not Others?

This injectable is designed and approved for so-called “dynamic” facial wrinkles. Dynamic creases are formed by the muscular contractions and skin folding that occurs when you make facial expressions. These facial movements occur when you smile, frown, furrow your brow, squint and laugh.

On the other hand, wrinkles caused by loss of volume beneath your skin must be treated with injectable fillers. Fillers replace the diminished fat and collagen beneath the skin. They thereby fill in and smooth out these types of wrinkles, which are not caused by the muscular contractions in your face.

How BOTOX® Cosmetic Works

BOTOX® Cosmetic works by softening the muscle activity causing the facial folds. The great thing about BOTOX® is that it is highly targeted: In the hands of an experienced injector like Dr. Cusick, only the intended area is treated without affecting the surrounding areas.

This effect is temporary. After 3-4 months, the muscles are fully capable of contracting the skin once again. Therefore, re-treatments are necessary to maintain the effect.

This injectable is a safe, FDA-approved formulation of the neurotoxin protein called botulinum toxin Type A. BOTOX® blocks the nerve signals that cause the muscle to contract and wrinkle the skin. The treatment itself is not unduly painful or uncomfortable. A very thin, fine needle is used, so while it’s normal to feel a pinch or slight pressure, the vast majority of patients don’t describe the injections as painful.

The injection site may be red, raised, sore and/or itchy for a few hours. You must not rub the area for 24 hours so that everything has a chance to settle. Don’t lay down or bend over for 4 hours. You can get back to normal activities right away.

How much does it cost? The cost varies based on the number of treatment areas, which dictates how many units of the injectable will be needed. Is it painful? Patients do not report pain, although a mild pinch is normal as the needle is inserted in the skin. It lasts only a moment.

How long do the results last? 3-4 months, at which point re-treatment is necessary.

Who Should Consider Botox?

Botox Cosmetic is indicated for the temporary correction of moderate to severe glabellar lines (frown lines) associated with corrugator and/or procerus muscle activity in patients 18 to 65 years of age. If you exhibit lines on the smooth part of your forehead above and between the brows (the glabella), you should consider treatment with Botox Cosmetic.

Botox Cosmetic is contraindicated if the patient exhibits an infection at the proposed injection site(s) and or has a known hypersensitivity to any botulinum toxin preparation or to any of the components in the formulation. Do not undergo Botox treatment if you are allergic to any of the ingredients or if you have experienced an allergic reaction to another product containing botulinum toxin. Ask your plastic surgeon about the possible side effects of Botox.

Botulinum toxins won’t fix all types of skin problems. For example, while it can diminish under-eye wrinkles, it can’t repair the damage caused by the sun. Your skin thickness, type, and the degree of wrinkling all play a role in determining the effectiveness of botulinum toxin. A qualified plastic surgeon is the best person to evaluate the appropriateness of this treatment for you personally.
